Enclomiphene vs TRT: Fertility, Results, Risks and Cost
Quick answer
TRT replaces testosterone; enclomiphene makes your body produce more. What that means for sperm counts, evidence, monitoring and cost.
Our top pick: T-TIME, From $69/mo. The lowest published price we track, with a licensed M.D.
Enclomiphene vs TRT comes down to one difference. Testosterone replacement therapy (TRT) puts testosterone into your body from outside. Enclomiphene is a pill that signals your own testes to make more. That difference drives almost everything else: fertility, which men qualify, how much evidence exists, and cost.
How each one works
TRT, given as gel, injections, patches or pills, raises blood testosterone directly. As the FDA's 2022 briefing explains, outside testosterone also tells the brain to cut LH and FSH, the hormones that drive the testes. Sperm production falls as a result.
Enclomiphene blocks estrogen's brake on the brain, so LH and FSH go up and the testes make more testosterone themselves. It only works if the testes can respond, which is why trials enrolled men with secondary hypogonadism: low testosterone with low or normal LH.
Fertility: the biggest difference
In two 16-week phase III trials of overweight men aged 18 to 60, both enclomiphene and testosterone gel raised testosterone. Enclomiphene kept sperm concentration in the normal range, while the gel group saw a marked drop in sperm production. A phase II trial found the same pattern.
The Endocrine Society recommends against starting testosterone therapy in men who want to father a child in the near term, as cited in the FDA briefing. The same document notes that most men who stop TRT see normal sperm production return within a year, and that hCG is the FDA-approved non-testosterone option for men with secondary hypogonadism who want fertility. Our article on hCG and Clomid together covers that route.
How strong is the evidence?
This is where TRT is ahead. Testosterone is FDA-approved for hypogonadism and has large, long trials behind it:
- The TRAVERSE trial of 5,246 men found no increase in major heart events versus placebo (7.0% vs 7.3%), but more atrial fibrillation, kidney injury and pulmonary embolism.
- The Testosterone Trials in men 65 and older found moderate improvement in sexual function and a small improvement in mood after a year.
- Enclomiphene trials lasted months, not years, and measured hormone levels and sperm rather than symptoms.
Sources: TRAVERSE, NEJM 2023 and Testosterone Trials, NEJM 2016. The European Medicines Agency refused an enclomiphene product in 2018, partly because the studies did not test whether symptoms such as libido or bone strength improved, and because of a blood clot signal.
Side effects and monitoring
Both need regular labs. TRT is known for raising hematocrit, shrinking the testes and suppressing fertility. Enclomiphene trials reported headache, hot flushes, nausea, muscle spasms and, rarely, blood clots; our enclomiphene side effects guide has the details. Either way, the AUA guideline says low testosterone should be confirmed with two early-morning tests below about 300 ng/dL before treatment.
Results: how fast and how much
In the 16-week phase III trials, enclomiphene and gel both restored testosterone to the normal range. In a small 2013 study, average testosterone after six months was similar on enclomiphene (525) and gel (545). Injections can push levels higher, but higher is not automatically better; clinicians aim for symptom relief with safe labs.
Cost
Telehealth enclomiphene programs we track start at about $69 a month at T-TIME and go up to $199 a month, before labs. For TRT, Keeps, for example, lists injectable testosterone at $199 a month including medication, baseline labs and the clinician visit. Use our enclomiphene and TRT cost calculator to compare a full year with labs, and see the provider comparison for each program's terms.
A typical monitoring plan
Programs differ, but most follow a similar rhythm for either treatment:
- Before starting: two morning testosterone tests, plus LH, estradiol, hematocrit and, depending on age, PSA.
- First follow-up: a repeat panel within the first few months to check the dose.
- Ongoing: periodic labs while on treatment, more often if hematocrit, estradiol or PSA move.
Ask any provider which of these labs are included in the price. On our provider comparison we list lab fees separately because they change the yearly total more than most people expect.
Can you take enclomiphene and TRT together?
Some clinics offer combined protocols, for example testosterone with a SERM to try to protect testicular function. We did not find large controlled trials of these combinations in the sources we reviewed, so this is a decision for a specialist who follows your labs closely.
Which fits whom
- Want children soon, or want to keep testicular function: enclomiphene, clomiphene or hCG are the options usually discussed.
- Primary hypogonadism (testes not responding): enclomiphene is unlikely to help, because it relies on the testes.
- Want the option with the longest track record and symptom data: TRT.
- Prefer a daily pill and no injections: enclomiphene, or an oral testosterone product.
FAQ
Is enclomiphene the same as TRT?
No. TRT replaces testosterone from outside the body. Enclomiphene raises your own production by increasing LH and FSH.
Does TRT make you infertile?
TRT lowers sperm production while you take it. Most men recover within a year after stopping, but anyone planning a family should discuss it before starting.
Is enclomiphene as effective as TRT?
For raising testosterone into the normal range, small trials found similar results to testosterone gel. TRT has far more evidence for symptom benefits and long-term safety.
Can I switch from TRT to enclomiphene?
Some men do, especially to restore fertility, and one early trial enrolled men coming off testosterone gel. A clinician should plan the switch and recheck labs.
Common misconception: is enclomiphene natural testosterone therapy with no risks?
It does make the body produce its own testosterone, but it is still a prescription drug with side effects. Trials reported blood clots in rare cases, and it is not FDA-approved.
Which is cheaper?
Prices overlap. Enclomiphene programs we track run about $69 to $199 a month, and labs add to both. Compare the 12-month total, not the monthly headline.
