GLP-1 guide for men
GLP-1 for men: semaglutide and tirzepatide options compared
GLP-1 medicines such as semaglutide (Wegovy, Ozempic) and tirzepatide (Zepbound, Mounjaro) are among the most effective weight-loss drugs available. For men, they also matter for testosterone: excess weight is one of the most common reasons for low T. This page compares what online programs charge, what the trials show, and the trade-offs between compounded and brand-name versions. Prices checked Nov 2, 2026.

How GLP-1 drugs work and what the trials show
Semaglutide copies the gut hormone GLP-1, which slows stomach emptying and signals fullness to the brain. Tirzepatide acts on GLP-1 and a second hormone, GIP. Both are taken as weekly injections in the trials below, alongside diet and activity advice. Results from these trials apply to the FDA-approved products; compounded and oral dissolving versions have not been tested in them.

What men should know before starting
- Testosterone often improves. Weight loss lifts testosterone in many men with obesity. In a small randomized trial of 25 men, semaglutide raised total testosterone without lowering sperm counts, which TRT did. If your low T persists after weight loss, look at enclomiphene or TRT with a clinician.
- Protect your muscle. Part of the weight lost on GLP-1 drugs is lean tissue. Strength training and enough protein are the first-line ways to keep it.
- Expect stomach side effects early. Nausea, diarrhea and constipation are common during dose increases and usually ease. Read our guide to GLP-1 side effects in men.
- It is long term. Weight tends to come back after stopping, so the yearly cost matters more than the first month.

What else T-TIME offers
T-TIME started as a men's testosterone clinic. Its best-known product is enclomiphene from $69 a month, our lowest-cost pick for men with confirmed low testosterone. The same intake also covers:
- Sermorelin
An oral option aimed at sleep, recovery and daily energy.
- LDN (low dose naltrexone)
An oral option aimed at inflammation, everyday pain and brain fog.
- OVERTIME
An ED treatment: troches that combine vardenafil and tadalafil.
- Charge
A daily chew that pairs enclomiphene with tadalafil.
Research and regulator sources
The trials, regulator pages and studies this guide relies on.
- FDA: Concerns with unapproved GLP-1 drugs used for weight loss
Compounded semaglutide and tirzepatide are not FDA-reviewed; FDA lists dosing errors, fake pharmacies and telehealth red flags, and had 990 and 730+ adverse event reports by May 2026.
- FDA: Policies for compounders as GLP-1 supply stabilizes
The semaglutide and tirzepatide shortages are resolved, so shortage-based compounding of copies of the approved injections has ended.
- STEP 1 trial, New England Journal of Medicine 2021 (semaglutide 2.4 mg)
1,961 adults without diabetes lost 14.9% of body weight on weekly semaglutide over 68 weeks vs 2.4% on placebo.
- SURMOUNT-1 trial, New England Journal of Medicine 2022 (tirzepatide)
Weekly tirzepatide 15 mg led to 20.9% average weight loss at 72 weeks vs 3.1% on placebo.
- SURMOUNT-5 trial, New England Journal of Medicine 2025 (tirzepatide vs semaglutide)
In 751 adults with obesity, tirzepatide cut weight by 20.2% vs 13.7% with semaglutide at 72 weeks.
- Endocrine Society (ENDO 2025): Anti-obesity drugs and testosterone in men
In 110 men on semaglutide, dulaglutide or tirzepatide, the share with normal testosterone rose from 53% to 77% with about 10% weight loss.
- Randomized trial of semaglutide vs TRT in men, Diabetes, Obesity and Metabolism 2025
In 25 men with obesity, type 2 diabetes and low T, both raised total testosterone; semaglutide improved sperm shape, while TRT lowered sperm counts.
- MedlinePlus (NIH): Semaglutide injection drug information
Carries a warning for thyroid tumors seen in rats; tell your doctor about pancreatitis, gallbladder or kidney disease.
GLP-1 FAQ
What is the cheapest GLP-1 for men?
Of the six online programs we checked on November 2, 2026, T-TIME had the lowest ongoing all-in price: compounded semaglutide at $99 a month at every dose, and tirzepatide at $447 per three months. Brand-name Wegovy pills start at $149 a month self-pay and rise with dose. Full GLP-1 cost breakdown
Does semaglutide raise testosterone in men?
It can, mostly through weight loss. In a 2025 Endocrine Society study of 110 men on GLP-1 drugs, the share with normal testosterone rose from 53% to 77% alongside about 10% weight loss. It is not a testosterone treatment, so men with confirmed low T still need a proper workup. GLP-1 side effects in men
Is compounded semaglutide the same as Wegovy?
No. It uses the same active ingredient, but it is prepared by a pharmacy for one patient and FDA does not review it for safety, effectiveness or quality. Wegovy and Zepbound are FDA-approved finished products. Compounded vs Wegovy and Zepbound
Semaglutide or tirzepatide: which works better?
In the SURMOUNT-5 head-to-head trial, tirzepatide led to more weight loss (20.2% vs 13.7% at 72 weeks). It also costs more at most clinics. Both mainly cause stomach side effects, and your clinician decides which fits your health.
What happens when you stop a GLP-1?
Most people regain weight. One year after stopping semaglutide in the STEP 1 extension, participants had regained about two-thirds of what they lost. Plan for long-term cost before you start.
Informational only, not medical advice. GLP-1 medicines are prescription drugs; a licensed clinician decides whether one is right for you. Do not use them with a personal or family history of medullary thyroid cancer or MEN 2.
